Client Agreement

I agree, by checking the box required, that I acknowledge the following and would like to initiate health coaching.

I am aware that:

1. Participation is voluntary and I may withdraw at any time by notifying my coach via email or phone.

2. The first coaching session will be 75 minutes and follow-up sessions will be 50 minutes.

3. Health coaching sessions will take place through either Zoom, FaceTime, or a phone call at a day and time that are mutually agreed upon.

4. No refunds or exchanges will be issued.

5. Health coaches are not healthcare providers and coaching does not replace seeing a licensed physician.

6. Health coaching is in no way to be construed or considered as psychological counseling or any type of therapy or medical intervention.

7. Health coaching results cannot be guaranteed.

8. I am fully responsible for the choices and decisions in my life and I am responsible for my own results.

9. My coach may release me from coaching for any reason, including but not limited to inappropriate conduct.

10. I agree to hold my coach free of all liability and responsibility for any actions or results created as a direct result or indirect result of specific referral or advice given by the coach. 11. My coach affirms that she will maintain strict confidentiality about all information I share. The only exception is if my coach has reasonable cause to believe there are threats of serious harm to myself or others.

12. This agreement will be governed by civil laws of The State of California (USA).

This agreement may be extended or terminated by mutual agreement.
